Introductioncystoskeleton which includes microbule, microfilament and intermediate is the specific structure of eucaryotic cell. Intermediate includes cytokeratin, vim-entin, desmin and nerves metaphysic. Cytokeratin ( CK) is the most extensive in intermediate filament. It is common for CK in covering epithelium and Vimentin (Vim) in glandular epithelium.BPH is one of the most common disease for old male and its attack rate has continuely risen recently in our country. It has been the most common disease in urinary surgery and brought severe physical and mental impact for many people. BPH is a accrescent disease essentially which can be expressed in two compositions;glandular organ and interstitial substance. It has been proved that BPH had different significance when it occured in interstitial substance or glandular organ.To observe the expressions and significance of cytoskeletal protein in interstitial substance or glandular organ of prostate, immunohistochemical investigation on two cytoskeletal protein CK34βE12 and Vim was performed on normal and accrescent prostate.Materials and MethodsSpecimen and case history;20 specimen of BPH were obtained at the middle point from bladder neck to caudal of caput gallinaginis which were confirmed BPH by patho - scopy. The age was 65 - 81, at average of 73. 68 ±4. 79;Thevolume of prostate determined by B - us was 24. 27 -108. 30cm3 ,at average of 45.49 ± 19. 51cm3;The IPSS was 14 - 34, at average of 21. 80 ± 6. 60. There were 5 NP with the age 26 -32 at average of 29.4 ±2.41 which were confirmed no patho - change by histology examination.Immunohistochemical SP method was used to detect the expression of CK34pE12 and Vim, after inhibition of endogenous peroxidase activity(3% hydrogen peroxide PBS solution for 20 min) , antigen retrieval was then performed by heating in a microwave oven in 10 raM citrate buffer for 10 min. After serum incubation, sections were incubated with rabbit monoclonal antibodies against CK34pE12 and Vim at 4TI for one night at a dilution of 1;100 respectovely, and another 10 min at RT to enhance incubation. Then the sections were incubated by biotinylated goat anti - rabbit secondf antibody at room temperature for 20 min, followed by being incubated with SP reagent at room temperature for 20 min. DAB was used as the chromogen for visualization. Finally, the sections were counterstained with hematoxylin, dehydrated and mounted.Decision of the result;Positive cells have yellow or buffy grains in their in-tracytoplasm. The positive cells of Vim and total cells were counted in 5 randomly selected fields under microscope at a 400 - fold magnification. The ratio of Vim - positive cells to total cells were calculated.The data were analyzed using SPSS for Windows 11.0 with a significant level of P<0.01.ResultsThe expressions of CK34[3E12 were seen in all specimen(BPH and NP). This antibody bind especially with endochylema of basal cells and its zonareac-tion enclose the gland with irregularity, linea aspera and masculine. Two groups have no difference because the basal cells in both groups expressed successively with a 100% rate.The expressions located in the endochylema of cellula intersitialis of Vim were also seen in all specimen. The group of BPH expressed more manifest than the group of NP and the two groups have significant differences (p <0.05).The positive rate of Vim has a significant negative correlation compared with the volume of prostate ( r = - 0. 640 ,p < 0. 01) , has a significant positive correlation compared with IPSS ( r = 0. 747 , p < 0. 01 ) , and has no correlation compared with the age of patients(r = 0. 026 ,p > 0. 05 ).DiscussionCystoskeleton resides generally in cells of organism with different forms which construct the supporting structure and caffolding protein of kytoplasm. At precent, the study of cytoskeletal protein was concentrate mainly in lung, gastrointestinal tract, liver, brain and so on but prostate.We studied the specimens of BPH and NP by immunohistochemistry through CK34pE12 and Vim and confirmed that the two cytoskeletal proteins expressed respectively on basal cells and interstitial cells. Our studies confirmed that trie expressions of cytoskeletal proteins CK34|3E12 which can mark specificly basal cells of prostate were no difference between BPH and NP. It has important significance for decision the changes of prostate that basal cells of prostatic carcinoma lossed completely.Our studies confirmed that Vim expressed in the interstitialcells of prostate and expressed excessively in BPH. The significant deviation of expression in different cases demonstrated that the expressions of Vim in BPH had some relations with the advancement of this disease. In the present study, the expressions of Vim in interstitialcells were correlately analyzed with different clinical parameter of the patients suffered with BPH. We found that the positive rate of Vim had a significant negative correlation compared with the volume of prostate, had a significant positive correlation compared with IPSS, and had no correlation compared with the age of patients. Some studies have confirmed that the proportion of interstitial substance tapered and the proportion of glandular organ, endepi-dermis, and glandular cavity increased gradually followed with the increase of the weight of prostate, which was consonance to the relations between the positive rate of Vim and the clinical parameter of BPH in our study. The dependability between the positive rate of Vim and the clinical parameter of BPH indi-cated that the expressions of Vim in interstitisl cells could deputy nicely the proportion of interstitisl cells in prostate gland. It has been proved that there had different significance when the accrementition was mainly interstitial substanqe or glandular organ, so it is important to discriminating the construction of BPH. As a detectable index, Vim can become a objective according in the study of the constituent ratio of prostate and has important significance for guiding clinical drug treatment.Conclussion1. Cytoskeletal protein CK34|3E12 and Vim expressed respectively on basal cells and interstitial cells of prostate with specificity. It is also proved that the expressions of CK34[3E12 had no difference in BPH and NP, but Vim expressed more excessive in BPH.2. CK34pE12 can mark basal cells of prostate specifically. It is helpful to discriminate the changes of prostate that if the basal cells exist by immunohisto-chemistry with CK34pE12.3. In the present study, the expressions of Vim in interstitialcells were cor-relately analyzed with different clinical parameter of the patients suffered with BPH. The expressions of Vim in interstitisl cells can deputy nicely the proportion of interstitisl cells in prostate gland, which can provide objective method and index for study the constituent ratio of prostate gland.
