Objective: To investigate the expression of monocyte chemoattractant protein-1﹙MCP-1﹚in IgA Nephropathy﹙IgAN﹚ and its relationship with the scope of histologic changes and laboratory parameters of renal function. And to study the influence of short-term treatment with steriod hormone.Methods: The levels of serum and urinary MCP-1 were detected by enzyme-linked immunosobent assay with the results to be compared. 38 cases with IgAN and 20 cases healthy individuals served as control group. Laboratory parameters of renal function such as serum and urinaryβ2-microglobulin﹙β2-MG﹚, 24h urine protein, and serum creatinine were measured in all IgAN patients. The scope of histological changes were determined. All these were measured associativity with MCP-1. 20 patients of IgAN were treated with prednisone of 1mg/kg/d for 2 weeks, pretreatment and post-treatment urinary MCP-1 and protein levels were measured and compared.Results: Urinary levels of MCP-1 in IgAN patients were siginficantly higher than those in control﹙P<0.01﹚ and were positively correlated with tubulointerstitial lesion, urinaryβ2-MG and 24h urine protein﹙P<0.05﹚. Serum MCP-1 levels remained similar to those of healthy group﹙P>0.05﹚. Urinary levels of MCP-1 decreased significantly after using steroid hormone﹙P<0.01﹚, however, there is no statistic decrease of 24h urine protein﹙P=0.055﹚.Conclusion: Measurement of urinary MCP-1 is a useful and noninvasive marker in evaluating tubulointerstial damage of IgAN patients. Using steroid hormone can decrease the level of urinary MCP-1, and thus could probably be a beneficial role in the prevention and therapy for tubulointerstial damage.
