Objective:Epilepsy is a common clinical syndrome. It is based on abnormal discharge of brain neurons caused by sudden, repeated and short-term central nervous system dysfunction in a chronic brain disease. It is a kind of originated in the brain, and repeated episodes of motor and sensory, autonomic nervous system, consciousness and mental state different levels of obstacles, it has variety of performance. Clinically, the initial onset is partial sure then with a sudden loss of consciousness, seizures of throwing himself, limb cramps, vomit, or watering in the mouth, and once wake up, as an ordinary people mainly. Its incidence rate is about 5‰. There probably have about six million patients with epilepsy in China. Epilepsy in children is a unique disease field, In patients with epilepsy, about 75% -80% onset in childhood. Children with epilepsy have complicated sub-type, disease process and individualized treatment plan. Most scholars believe that neural immune regulatory network imbalance Involved in the pathogenesis of epilepsy. In the complex neural networks, there are two-way adjustment between nervous system and immune system. But it is complicated in epilepsy patient's immune pathogenesis. Different duration type, seizure frequency, the use of drugs, age, sex and cause of the different, and many other factors maybe affect the level of cytokine secretion in vivo. In this study, through the detection of cell-mediated immunity, humoral immunity and adhesion molecule 1 in children who are with primary partial epilepsy, to investigate the changes of immunization levels and the influence of new antiepileptic drug oxcarbazepine, topiramate on immune ability in children with epilepsy, and to understand the pathogenesis of epilepsy, also the new possible mechanism of action of antiepileptic drugs, since it can lead a new approach in treatmen. Methods:This study belongs to experimental case-control study, 54 newly diagnosed primary partial epileptics, who were treated at neurology clinics and wards of Hebei Children's Hospital from August 2008 to September 2009, were chosen as epilepsy group, while another 21 healthy subjects cases from Children Health care department of Hebei Children's Hospital were chosen as normal control group. The epilepsy group were divided into topiramate group and oxcarbazepine group with randomly number method. Blood samples were collected respectively from the epilepsy group before taking medication, 3 months and 6 months after taking medication, while blood samples were collected only once from the healthy normal control group. 2ml blood without anticoagulant and 1ml with sodium citrate blood anticoagulant were collected from the children with empty stomach on early morning. Applying automatic biochemical analyzer (Hitachi 7060 automatic biochemical analyzer), to detectde levels of serum immunoglobulins lgG, IgA, IgM through immune turbidimetry, and flow cytometry (USA BECKMAN COULTER company, model Epics XL) to detected levels of CD3, CD4, CD8 and adhesion molecule -1 (ICAM-1).The statistical analysis SPSS13.0 statistics software was used to carry on statistics processing, p<0.05(double side) had statistical significance.Result:1 Compared with the normal control group,the levels of IgA, IgG, ICAM-1, CD8 levels increased, CD3,CD4 levels increased in the blood of children with epilepsy before treatment, there was significantly statistical difference between them(P<0.05),whereas level of IgM had no statistical differences with that of normal control group.2 After 3 months treatment with topiramate, IgG,ICAM-1 levels decreased, there was Significantly statistical difference with before treatment group, the increased of IgA compared with before treatment group and the normal control group was Significantly statistical difference, while CD3,CD4 levels increased, CD8 level decreased, the change of IgM had no statistical differences with before treatment group and the normal control group, After 6 months treatment with topiramate, the levels of IgA,IgG,CD8,ICAM-1 level decrease, there was significantly statistical difference with before treatment group , and there was no statistical differences with the normal control group. while CD3,CD4 levels increased, IgM had no statistical differences with before treatment group and the normal control group, there was no statistical differences between 3 months treatment and 6 months after treatment with topiramate.3 After 3 months treatment with Oxcarbazepine, levels of IgA,IgG,CD8 decreased, CD3 levels increased, there was Significantly statistical difference with before treatment group and no statistical differences with the normal control group, The IgM, increased of CD4, decreased of ICAM-1 compared with before treatment group and the normal control group had no statistical differences, After 6 months treatment with Oxcarbazepine, IgA,IgG,CD8, ICAM-1 levels decreased, CD4 levels increased, there had Significantly statistical difference with before treatment group and no statistical differences with the normal control group, The IgM, increased of CD3, compared with before treatment group and the normal control group had no statistical differences, there was no statistical difference between 3 months treatment and 6 months treatment with oxcarbazepine group.4 There was no statistical differences in immune parameters between before treatment, 3 months treatment and 6 months after treatment with antiepileptic drug topiramate or oxcarbazepine.Conclusion:Cell-mediated immunity and humoral immunity functions were disordered in Children with Primary partial epilepsy before taking antiepileptic drugs, The mainly manifested was the increased of IgA,IgG,CD8,CD54 and the decreased of CD3,CD4,CD4/CD8; The increased of blood cell intercellular adhesion molecule-1 in epilepsy children suggested that it was possible inflammatory cytokines involved in the pathogenesis of epilepsy. There was a change in Immune function in children with epilepsy compared with itself before and after use of the new antiepileptic drug topiramate and oxcarbazepine monotherapy. Description of that the immune function of children with epilepsy had a regulate effect when antiepileptic drugs to control epileptic seizures. There was no Significantly difference between topiramate and oxcarbazepine treatment groups. There was abnormal immune function in children with epilepsy, it suggested that we should be pay attention to improving the immune function at the same time of the clinical treatment of the antiepileptic, to improved the quality of life of children and improved the prognosis.
