National long-term reform and development Plan (2010-2020) points out the strategic theme of the education reform:insist on people-oriented mentality and promote quality education. The core theme is to solve the significant problem that how to cultivate the personnel and the key is to cultivate all students to promote comprehensively and adhere to the unity of the comprehensive development and personality development. Under the situation of the new education reform round, the author considers that the implementation of personalization education is the most important chapter in Education Reform Innovation to solve the problem of comprehensive development under quality education and making students to show their own personality in the overall development.Teaching is the central task of school education, the classroom is the main front to cultivate students, and the teachers are the guider for students to develop. Therefore, respecting highly for personality characteristics of teachers and students in teaching and learning process, enhancing the effectiveness of teaching and learning activities and promoting the academic and personal development of students have been an significant value of the personalization teaching. In this Paper, the author has united the theoretic and practical foods of personalization education at home and abroad, recalled the modern personalization teaching modes,made innovation in personalization education based on the original teaching mode and put forward a new personalized teaching mode——team-chain teaching mode:Teachers provide axis or guide the students to build the knowledge axis, thinking axis and emotional axis and link the chain-film of the mathematical knowledge, thinking and emotion into the chain. This teaching model fully inspired the personality and special skills of the students. The students can put forward problems after discussing to each other and solve them and obtain the valuable research results through independent thinking, cooperation and exchanges to set up teaching modem.Meanwhile, the author has also designed the specific teaching cases, aiming at exploring in teaching practice of this teaching mode in Mathematics teaching. Therefore, the paper has a theoretical analysis and practice exploration on the mathematics teaching in education reform.
