Guidance case is a new concept proposed in the1990s. It is the native "creature" of Chinese class teaching reform. In recent years learning-case teaching is a new teaching mode with Guided learning plan as a carrier to promote students’ learning initiative. The use of learning case makes the mathematics teaching in high school undergo tremendous changes. It has changed teachers’teaching method and students’learning approaches, such as it makes a teacher focus on students’ learning ability rather than pays attention to his own teaching;a teacher become a guider of students instead of one who likes chalking and talking. Students also change from the previous "passive study mode" into "explore actively mode "etc.City G of Jiangsu province has implemented respectively the "Three case six links" teaching mode,"six module Constructivist Classroom" mode and "construction of ecological classroom" mode since2009,which are teaching modes with the "Guided learning plan" as a carrier. In this context, this paper surveys both the teachers and students with interviews and questionnaires. The result of the statistics analyzed show that the majority of students and teachers have approved the teaching mode with Guidance case. Many students and teachers are willing to use "Guided learning plan" in mathematics problem solving, learning proposition and to use "Guided learning plan" in review class. The vast majority of students believe that if the "Guidance cases" design is reasonable, it will not increase their learning burden. The use of "Guided learning plan" is of great help to students’preview and study in class. For example, it helps the students to develop good learning habits and can improve their learning efficiency and learning achievements, etc. Teachers and students also mentioned confusions and problems in the using process of "Guided learning plan". According to the "Guidance case" typesetting, distribution and questions, they have each airs their own views.According to the problems and suggestions pointed out by teachers and students in the investigation, this essay focuses on the design and the writing of "Guided learning plan". The author respectively introduces the part of the "Guidance cases", the design process of concept learning, proposition learning and problem solving study. This essay also put forward some suggestions for the future use in mathematics teaching so as to provide some useful reference for those who want to do researches on this topic.
