Based on The Three Methods of Professional social work (case, group and community) andthe principle of voluntariness, I provided door interview and professional social work services toalmost40empty nesters,who live in Beijing Chaoyang district Tuanjiehu and Chaowaistreet,describing and analyzing the present situation of home-based care in this area.According to the investigation, I found that both the empty nesters, whose reimbursementproportion of medical insurance with relatively high percentage, and the poor ones who can onlylive by their social security payments, had the problems such as:the dissatisfaction of medicaland health services; lack of spiritual and cultural life; deficiency of communication with theoutside and proper entertainment activities, which lead to the emerge of negative emotion likeloneliness or anxiety. Without meeting the needs mentioned above, elder people may not enjoythemselves in their later life, let alone physical and mental health. Thus, the solution to thisphenomenon is a problem of developing home-based care for the aged pattern. The evaluationsto the elder people in Tuanjiehu, based on the theory of Maslow’s need Hierachy Theory, as wellas the guide of Rational Emotive Therapy and Social support network theory, was provided inprofessional services to empty nesters, so was a certain social work intervention programme. Itnot only provided the troubled elder people in specialized needs with personalized interventionof case study, which solve the problem and puzzlement of the elder people specifically in a shorttime, but enhanced the integrated efficiency of social work by carrying out group activities forpart of the elder people with common demand to meet their needs. Workers mobilized socialresources and carried out community service activities to expand service object and targetbeneficiaries, developed potential egotism and needs by large activities of community group toprovide clients and team members for the development of case and group study,cyclic and round,made the social work intervention of empty-nesters home-based care form a system ofecosphere,and provided favorable technical support for constructing new type and professional“no wall”nursing homes, when most empty-nesters can’t meet their needs in this area.This paper involved individual cases intervention and specific services of several elderpeople during the internship. There was also some difficulties and problems during the serviceprocess, such as personalized conflict with equality, transference and countertransference,low awareness of social work,insufficient capacity of linked resources of social workers,unsoundvolunteer service mechanism and job indolence caused by low income. This paper analyzed anddiscussed the case from these six aspects,and try to propose a certain solution,and hope toprovide reference and guidance for later social work intervention of empty-nesters home-basedcare,as well as carrying out community service for the Elder on a large scale by linkingresources,combing the methods of social work, case group and community in an organic way. Ihope that the integration from my practical experience will be helpful for establishing generalpattern of home-based care for the aged, a new type of‘no walls’nursing home. Once thehouse‘ecosystem’is formed based on the community, guided by social workers,assisted onmedical personnels, as well as integrated services by other service providers,it willunquestionably provide an intergrated household-supporting pattern which has strongimplements,simple operation,with high quality and affect.Thus ease the pressure and burden ofgovernment and society and let empty-nesters reassuring for house without worries.
