China has a lot of theoretical research on community home-based care, but thepractice of home care in each region is still in its exploratory phase. Home-based careproject in Changying community is one of the many practical modes, research on itcould help explore community home care practice model. So the author want to exploreeffective ways of home care and social work intervention through case study ofHome-based care project in Changying communityIn the first chapter the author clarifies the four concepts---Home-based care,Empty nesters, Community and Case Work. The second chapter describes the situationand needs of home-based care in Changying community through100questionnaires on100empty nesters. Combined with the observation in Changying community, the authoralso points out problems in the elderly’s home-based care: low utilization rate ofhardware for the elderly; Poor efficiency on the implementation of welfare policy for theelderly; weak professional services; lack of casework for different elderly. The thirdchapter presents a specific casework in home-based care project of Changyingcommunity, which was carried out by the firm specified on the social work intervention.The fourth chapter concludes: when government increase hardware investment in agingcommunity, these communities are not equipped with adequate software resources---professional social workers and project funds; poor efficiency on the implementation ofwelfare policy for the elderly; weak professional services; lack of casework for differentelderly. Therefore, the author propose the following policy recommendations. Firstly,implement on-demand services, rational allocation of pension resources; Secondly,emphasize the introduction of non-governmental organizations in community work and adiversified source of home-based care community support. Thirdly, the governmentshould strengthen human resource development in home-based care services,emphasizing the importance of professional staff.
