Comparative Study On The Effect Of Two Kinds Of Vitamin D Therapies And The Survey On The Nutritional Status Of Vitamin D Among Women Over40Years Old In Chengdu

Objective: To evaluate the nutritional status of vitamin D among women over40years oldduring winter in Chengdu. In order to know exactly the difference between the vitamin D2and D3injection in the treatment of vitamin D deficiency or insufficiency, and then provide abasis for the supplement of vitamin D.Methods:123healthy women aged over40had their vein blood sample taken in themorning to test the25-hydroxy vitamin D (25(OH)VD) in their serum in November,2012.94of them, who were proved to be vitamin D deficiency or insufficiency, were divided intotwo groups randomly. Group1, also named group vitamin D2, was given vitamin D2injectionof7.5mg (300000units) intramuscularly once every two weeks, a total of four injections. Atthe same time, group2, also named group vitamin D3, was given vitamin D3injection of7.5mg (300000units) intramuscularly once every two weeks, a total of four injections. Bothgroups tested calcium, phosphorus, magnesium, PTH,25(OH)VD and BAP two weeks aftertheir last injection.Results: In winter (November), the25(OH)VD level in the serum of women over40yearsold in Chengdu was15.92±7.02ng/ml,75.60%of them as a lack of vitamin D(25(OH)VD<20ng/ml),22.00%of them as insufficiency (20ng/ml≤25(OH)VD<30ng/ml).Only2.40%of them as sufficiency. There was no significant difference between group age <50and group age≥50(P>0.05).(1)In the group vitamin D2, the level of25(OH)VD increased after the therapy(20.69±7.87pg/ml vs14.72±5.78pg/ml,P<0.05), while the level of PTH decreased after theirtherapy(30.49±15.79pg/ml vs37.06±14.83pg/ml,P<0.05),the level of serum calciumincreased after therapy(2.29±0.12mmo/l vs2.23±0.08mmo/l,P<0.05), the level of serumphosphorus increased after the therapy(1.31±0.13mmol/l vs1.15±0.13mmol/l,P<0.05); TheMg、BAP differences were no significant between before and after treatment in bothgroups(P>0.05).(2)In the group vitamin D3, the level of25(OH)VD increased significantlyafter intervention(47.8±14.77ng/ml vs15.15±6.59ng/ml,P<0.05), while the level of PTH decreased(34.02±23.33pg/ml vs43.39±16.46pg/ml,P<0.05), the level of serum calciumincreased after therapy(2.27±0.07mmo/l vs2.23±0.08mmo/l,P<0.05), the level of serumphosphorus increased after the therapy(1.25±0.14mmol/l vs1.18±0.13mmol/l,P<0.05), whilethe level of serum magnesium and BAP has no significant difference compare to the levelbefore the therapy(P>0.05).(3)After the intervention, compared the level of serum25(OH)VD of the group vitamin D2with the group vitamin D3, there was a significantstatistical difference(47.8±14.77ng/ml vs20.69±7.87ng/ml,P<0.05). There was nosignificant difference between the two groups on the levels of PTH, BAP, calcium,magnesium and phosphorus(P>0.05).(4)Each injection of vitamin D7.5mg, a total of4injections of vitamin D3cost about5.20yuan, which means the increase of every more1ng/ml of25(OH)VD costs0.16yuan. At the same time, a total of4injections of vitamin D2costs151.2yuan, which means the increase of every more1ng/ml of25(OH)VD costs25.33yuan.(5)During the treatment, all94subjects didn’t suffer from any vitamin D intoxication ordrug-related side effect.Conclusion: The fact that the deficiency of vitamin D, which is quite serious, may be verycommon among women over40years old. Both vitamin D2and vitamin D3can effectivelyimprove the level of25(OH)VD、Ca and P and reduce the level of PTH, but without BAP.Both the8weeks’ therapies of Vitamin D2injection (300000u every2weeks) and vitamin D3injection (300000u/2weeks) are safe. Vitamin D3may have a more effective role inimproving the level of25(OH)VD in serum than vitamin D2. Also it is more economic in theapplication of vitamin D3in the treatment of vitamin D deficiency.
