| Objective:To evaluate the effect of local infiltration anesthesia with rapivacaine and preemptive analgesia with dezocine and the two drugs use together on healing of incision and the expression of GM-CSF.Background:Local anesthesia on the site of surgical incision is a common method of postoperative analgesia, and the concept of preemptive analgesia is also widely used in clinical anesthesia.Studies have shown that local anesthesia with 0.5% ropivacaine can promote the expression of GM-CSF and the healing of wound, Dezocine excited as a opioid receptor antagonist mixed excitement with strong analgesic effect and less side effects used in pre-clinical anesthesia for preoperative or postoperative analgesia, but its impact on the expression of GM-CSF and the healing of the wound is not clear.Methods:Using full-thickness skin damage in mice model. To build a skin damage and the size is 0.8cm x 0.8 cm in the center of the back near caudalafter intraperitoneal anesthesia. And before the modeling wound of 160 healthy and clean male mice, single cages way, turn each cage number respectively, No. 1-160, then in strict accordance with the random number table, the mice were randomly divided into five groups(n = 40): normal saline group(group NS),0.5% ropivacaine group(group L),dezocine(5mg/kg) group(group D),0.5% ropivacaine and dezocine(5mg/kg)group(group Y). Then each major group is divided into four groupst with 8 mice respectively.0.5% ropivacaine 1ml/kg was injected subcutaneously 10 min before incision in group L,anddezocine 5ml/kg was injected by intraperioneal injection 10 min before incision in group D,and each dose combination of two drugs in group Y,while the equal volume of normal saline was given instead in the no injection sections of the four groups. To cut tissue at the wound in the 3,5, and 7 days after injury,Determination of GM-CSF expression in the wound by ELISA, and detects each respective samples total protein content using the BCA protein quantitation method,the value measured by ELISA than the resulting value of its respective BCA represents the concentration of GM-CSF,and to measure the rate of wound healing; To detect pathological changes of the wound tissue in 5 and 10 days; Eight mice of the four groups feeding until wound healing to observe the wound healing time.Result:(1) ELISA assay results: After 3d,5d and 7d of full-thickness skin damage, the expression of L group and D group mice with GM-CSF was significantly higher than the NS group(p<0.05);And after the comparison between L and D group the difference is not statistically significant,the GM-CSF expression of Y group was higher than that in group D and group L(p<0.05).(2)Pathology results:Fifth days after operation: Group NS:Capillaries between endoplasmic fewer fibroblasts, inflammatory cell infiltration; group L :A large number of inflammatory cells, a small amount of interstitial fibroblasts; group D: A large number of inflammatory cells, a small number of fibroblasts; group Y: A small amount of collagen fibers,infiltration of inflammatory cells.The 10 th day after-operation: Group NS: squamous epithelial hyperplasia on the sueface, a small amount of collagen fibers, showing a small amount of inflammatory cells; group L: squamous epithelial thickening on the surface, a large number of collagen fibers; group D: squamous cell hyperplasia on the surface, a large number of interstitial collagen fibers; group Y: Surface squamous epithelium thickened collagen fibers increased significantly.(3) The rate of wound healing: the rate of wound healing after surgery were sacrificed on days 3, 7, 10 about 8 mice in each group, L group and D group mice was significantly higher than the NS group(p<0.05); And after the comparison between L and D group the difference is not statistically significant, the rate of wound healing of Y group was higher than that in group D and group L(p<0.05).(4)The time of wound healing: the time ofwound healing of L group and D group mice was significantly higher than the NS group(p<0.05);And after the comparison between L and D group the difference is not statistically significant,the time of wound healing of Y group was higher than that in group D and group L(p<0.05).Conclusion:Local infiltration anesthesia with rapivacaine and preemptive analgesia with dezocine can promote the expression of GM-CSF and wound healing of incision in mice.Each dose combination of two drugs had better effect,while the difference of the healing time,of the rate of wound healing and of the GM-CSF concentration were not statistically significant between local infiltration anesthesia with rapivacaine and preemptive analgesia with dezocine. |
