Objective To evaluate the clinical effects Liuzijue combined with resistance training on body mass index, lung function, dyspnea, exercise capacity, BO DE index and quality of life in patients with stable chronic obstructive pulmonarydisease(COPD). Compared with Liuzijue exercise, and to evaluate the effect of differences of treatments. To seek a cheap, convenient, easy rehabilitation program which well worked in disease prevention to apply to long-term home self- management rehabilitation training.Method A total of 60 COPD patients were recruited from Yue- Yang hospital of Shanghai University of Traditional C hinese Medicine in March 2013 to March 2014 form respiratory ward, lung function room and outpatient clinics, The inclusion criteria meet the diagnostic criteria of Global Initiative for Chronic Obstructive Lung Disease and classificationof grade II and III. The subjects were randomly divided into usual medical care(control group), Liuzijue exercise combined with usual medical care(Liuzijue group), Liuzijue combined with resistance training and usual medical care(combined exercise group), each group had 20 cases. Before start of study, all patients took the symptom limited treadmill exercise testing to evaluate cardiopulmonary function in Dynamic ECG Room, and recorded the maximum heart rate. Liuzijue group and combined exercise group were trained in six mouths, 6 times a week, each time 60 minutes. The target of Liuzijue was 60-80% of maximum heart rate. Patient in the combined exercise group took Liuzijue plus resistance trainingin four actions, each action had two groups. The target strength of resistance training was 50-60%1RM of target muscle group, 1-3 minutes between each group, and 3 times a week. During the intervention, we required our patients to fill in exercising journal and arranged for visitors regular follow-up by the wayof outpatient service, or telephone. Recording and analyzing body mass index, lung function, dyspnea, exercise capacity, BODE index and quality of life. Q uality of life, dyspnea symptom scores were tested with relevant scale. BODE index according to the method proposed by Celli et al. Exercise tolerance was tested with six- minute walk distance, Lung function was evaluated by lung function test instrument. The final data was analysed by statistical software SPSS 20.0.Results 1 Body Mass Index After the six months experiment, the three groups related body mass index had no significantdifferencecompared itself before and after experiment; there was no statistically between this three groups onbody mass index. 2 Lung Function After the six months experiment, the combined exercise group and Liuzijue grouprelated parameters of lung function FEV1 pred were significantlyimproved compared with before intervention, the difference has highly statisticalsignificance(P<0.01), Statisticallysignificant changes in FEV1/FVC% among two groups(P<0.05); There were no significantdifference in FEV1%pred and FEV1/FVC% between three groups after intervention(P>0.05). 3 Dyspnea After the six months experiment, both the combined exercise groupand Liuzijue group’ s m MRC dyspnea score were significantly decreased compared with before intervention, the difference has highly statisticalsignificance(P<0.01); The comparsion between groups after intervention, the combined exercise group and Liuzijue group of m MRC dyspnea score were significantly decreased compared with control group, the difference has highly statisticalsignificance(P<0.01), no statistically significant differencewas found in m MRC dyspnea score between the combined exercise groupand Liuzijue group(P>0.05). 4 Exercise Capacity After the six months experiment, both the combined exercise groupand Liuzijue group’s 6MWD were significantly improved compared with before intervention, the difference has highly statisticalsignificance(P<0.01); The comparsion between groups after intervention, the combined exercise groupand Liuzijue group of 6MWD were significantly improved compared with control group, the difference has highly statisticalsignificance(P<0.01), the combined exercise groupobviously improved than Liuzijue group in 6MWD, but the differencewas no statistically significantbetween groups(P>0.05). 5 BODE Index After the six months experiment, both the combined exercise group and Liuzijue group’s BODE index were significantly decreased compared with before intervention, the difference has highly statistical significance(P<0.01); The comparsion between groups after intervention, the combined exercise group and Liuzijue group of BODE index were significantly decreased compared with control group, the difference has statistical significance(P<0.05), the combined exercise group obviously improved than Liuzijue group in BODE index, but the difference was no statistically significant between groups(P>0.05). 6 Quality of Life After the six months experiment, both the combined exercise groupand Liuzijue group’ s each factor score of the quality of life were significantly improved compared with before intervention, the difference has highly statisticalsignificance(P<0.01); The comparsion between groups after intervention, the combined exercise groupand Liuzijue group of each factor score of the quality of life were significantly improved compared with control group,the difference has highly statistical significance(P<0.01), the combined exercise group obviously improved than Liuzijue group in symptomscore of the quality of life, the difference has statistical significance(P<0.05), and other factor score of the quality of life was no statistically significantbetween groups.Conclusion 1.The combined exercise groupand Liuzijue group can effectively improve lung function, dyspnea, exercise capacity, BODE index and quality of life in patient with stable COPD, but the body mass index have no significant effect. 2. Liuzijue combined with resistance training is a safe and effective method in the treatment of stable COPD. Liuzijue combined with resistance training obviously improved than Liuzijue exercise in symptom score, the change trend of other outcome outcome measures was more obvious, but there were no statistically significant between groups. So Liuzijue combined with resistance training could be used as a more effective method for community and home rehabilitation on COPD.
