This study is based on related theory of chronic diseases and health management by collecting teachers' health examination results and death causes of dead teachers in a university in Tianjin, analyzing of the main trends, and changing law and the harm degree of the onset of chronic diseases harmful to the staff. Meanwhile by combining with the university teachers' health questionnaire analysis, this research is to explore the main high-risk factors influencing the university teachers' health, determining the key crowd of health interventions, and finally putting forward the targeted health management measures. The main conclusions are as follows:Due to chronic disease, the health status of a university teacher is not optimistic. Physical examination results of 4295 senior teacher for four consecutive years show that university teachers have higher prevalence of chronic diseases than others. The 8 diseases teachers suffer most are all chronic diseases. The prevalence rate from high to low are hyperlipidemia, fatty liver, hypertension and high blood pressure values, ECG abnormalities, overweight and obesity, high blood uric acid, liver and kidney cysts, chronic pharyngitis. The prevalence of male is higher than that of female when it comes to dyslipidemia, fatty liver, overweight and obesity. The incidence of hyperlipidemias, which is increased with the increase of age, is normally distributed. But the incidence of fatty liver and obesity increases with age at first and then reduces latter.Teachers' average age of death of a college is 79.87 years old. average age of death of male is 78.2,which is lower than 81.19, which is a life expectancy of this area; College Teachers' mortality is 6.46 ‰, mortality of male is higher than that of female;93% death of college teachers is due to chronic diseases,among which 95% death are the top five disease; the cause of death in women are top 5 disease listed above, while the cause of death of male is various; cause of death in women is mostly cancer and respiratory diseases for male stroke and cardiovascular disease is higher except malignant tumors.This research finds that high work pressure, little sleep time,lack of exercise, physical overdraft are common among the college teachers and this is even worse among the techers aged from 45-55.the the main problems to the health of teachers; Smoking in college teachers, occupies a large proportion. Smoking case has obviously positive correlation with chronic diseases.The statistical analysis and research on the basis of above, put forward the countermeasures to strengthen university teachers' health management of chronic diseases and chronic disease health management model to the faculty are discussed in this paper. The main contents include:(1) according to the current chronnic diseases state of the college teachers,propose from the side of administrators and teachers of college;(2) The standardized operation flow of chronic disease health management for college teachers is proposed.,including establish personal electronic health records, Chronic disease risk assessment,create healthy programs, dynamic tracking and monitoring Health intervention for different population classifications this paper discusses the management mode for university teachers' chronic health conditions and chronic disease risk assessment,health intervention, and dynamic tracing monitoring methods; The health management division and service object are dynamically communicating on the standard system platform to complete each management project and quantify the quality of the assessment;(3) propose a concrete scheme of chronic disease health management of a college and a university.
