Since the "national student physical health standard(revised 2014)" revision completed,the "standard" has been implemented in schools across the country for 2 years,the effect of the implementation of how to be inspected,this research will be the "standard" function according to in order to achieve the purpose of the incentive oriented education,the establishment of the "standard" implementation effect evaluation the index system,to achieve the effect of the implementation of the "standard" will be incorporated into the government,education and school evaluation theory and the practice foundation.This paper constructs the evaluation index system of implementation effect of "standard" through literature and expert consultation,the 6 counties of Ningxia Province,a total of twelve primary and 326 students of senior high school has been verified in the investigation questionnaire,Check the validity of the questionnaire.Through the literature accumulation,the relevant policy support and expert evaluation,the paper establishes the theoretical framework of the evaluation of the implementation effect of the standard,which includes 3 first level indicators,namely,educational incentives,feedback evaluation,and guide training.7 level two indexes,namely the "standard" implementation of the relevant documents;the "standard" test carried out;students of physical education and extracurricular exercise and cognitive interest;"standard" to the "standard" test data;the evaluation results of individual feedback;students' physique and health;cultivate students physical exercise habits and behavior.Three indicators constitute the contents of the questionnaire,the questionnaire includes a student questionnaire,teacher questionnaire,the questionnaire,the questionnaire has a total of 6 dimensions,does not include the dimension of the "standard" test data reported.This paper tests the student questionnaire,the research through five rounds of investigation,using item discrimination analysis,he and item total correlation analysis,factor analysis,reliability analysis and test method,"" standard "implementation effect assessment questionnaire(questionnaire)".After the test,the theoretical framework of the questionnaire is established,which contains 6 dimensions.The questionnaire contains 40 items.Each entry of the questionnaire was resolved value reached 0.05 level,each item in the corresponding level item total correlation coefficient is greater than 0.3,each item item total correlation coefficients were all greater than he correlation coefficient,factor analysis 3 factors and the division result associated with each dimension of coefficient test results,and design a with the questionnaire of 3 factors construct the basic line,reflecting the questionnaire has good construct validity.The various dimensions of the questionnaire and the total questionnaire internal consistency reliability coefficients were between 0.867-0.943,the questionnaire split half reliability coefficient between 0.839-0.942,in a good level,the questionnaire has good stability.
