A review of the current evaluation methods of middle school physical education class, more is use the traditional final physical test to carry out a summative evaluation for students, evaluation subject is single. The main function of the traditional way of evaluation is screening and selecting students, it ignores the emotional experience of students and the cultivation of students' comprehensive quality in the process of physical education, and it can't effectively reflect the students' mastery of sports and health knowledge, and dampened the enthusiasm of students' physical exercise and the possibility of all-round development.In order to form a more perfect physical education curriculum evaluation system,according to the new curriculum requirements,by combining with the qualitative evaluation and quantitative evaluation, formative assessment and summative assessment,relative evaluation and absolute evaluation, student self-assessment, peer evaluation,teacher evaluation, it developed the basic principles which integrated PE and health curriculum standards into the whole process of middle school sports academic evaluation, and developed the middle school sports academic evaluation pattern. And then, through literature search and expert interviews,preliminary determine the indicators of academic achievement evaluation system, then the preliminary evaluation index system was made into the questionnaire, released to the discipline experts,experienced sports teachers and students. Finally, it determined five first level indexes and seventeen secondary indexs through the data analysis. The first level indexes are sports participation, sports skills, good health, mental health and Social adaptation and the secondary indexs are the attitude of participate in sports activities, the activity of participate in sports activities, the exercise habit, attendance rate, acquire basic knowledge of sports, motor skill mastery, physical security, body function, body shape,disease status, the consciousness of focus on the body and health, ability to build self-esteem, the ability to regulate emotions,a strong will quality, interpersonal relationships, the situation of cooperation with people in the activities, obtained sports and health knowledge from the society.The physical education curriculum academic performance evaluation model and evaluation system are researched by experimental research method, questionnaire survey method and interview method, the experimental group used the current evaluation system and the control group used the traditional evaluation method, the initial level of the two groups of students is comparative and the same teacher is used. The results show that the evaluation system and evaluation method of the study established have been approved and endorsed by the experimental group students. The questionnaire revealed that the degree of satisfaction, learning attitude and participation in physical exercise and the cooperation ability and interpersonal relationship of the students in the experimental group were higher than the control group who using the traditional evaluation method.Spss statistical results showed that there were significant differences in the two groups of students' physical fitness test scores, compared with the control group, the test scores of the experimental group were significantly higher than the control group.The physical education curriculum academic performance evaluation model and evaluation system are researched by experimental research method, questionnaire survey method and interview method, the experimental group used the current evaluation system and the control group used the traditional evaluation method, the initial level of the two groups of students is comparative and the same teacher is used. As a physical education teacher, we should continue to sum up and reflect in teaching and strive to make the evaluation system more scientific, comprehensive and reasonable, make it more widely used in sports teaching.
