The Efficacy Annalysis Of Thymus Expanding Resection Treatment For Myasthenia Gravis With Thymomas

Objective: To investigate the effect of thymus expanding resection treatment on different histopathologic types of WHO,the clinical efficacy of Massaoka's clinical staging thymoma with myasthenia gravis,the relation of WHO classification of thymoma,Maosaoka staging and myasthenia gravis,Myasthenia crisis related.Methods: The clinical and pathological data of 101 thymoma patients who underwent thymoma resection in the First Affiliated Hospital of Dalian Medical University from January 2005 to December 2015 were collected and retrospectivelyreviewed.The pathological classification of tumor was based on 2004 WHO classification.The therapeutic effect of thymoma resection on thymoma associated with different WHO types and Maosaoka clinical stage with myasthenia gravis was analyzed.Compare the WHO histological classification,Maosaoka staging with Myasthenia crisis,the correlation between the occurrence of myasthenia gravis crisis.Using SPSS19.0 for statistical analysis,take P <0.05 as the difference was statistically significant.Results:(1)In 49 cases of thymoma patients with myasthenia gravis,the effective rate of type A thymoma resection was 100%,the effective rate of AB type was 85.71%,the effective rate of type B1 was 100%,the effective rate of type B2(66.67%),the effective rate of type B3 was 50%,the effective rate of type C was 0%,the difference was statistically significant(P <0.05).The efficacy of A?AB?B1?B2 types thymoma resection is better than other types.(2)Among the 49 patients with thymoma,the effective rate of stage ? thymectomy was 86.37%,the effective rate of stage ? was76.47%,the effective rate of stage ? was 62.5%,the effective rate of stage ? was0%,the difference was Statistical significance(P <0.05),Maosaoka stage ?,?thymoma with myasthenia gravis patients with thymectomy is better.The efficacy of Maosaoka stage ?,? thymoma resection is better than other stage.(3)In 49 cases of thymoma patients with myasthenia gravis,the effective rate of Osserman type ?thymoma resection was 78.57%,the effective rate of type ? a was 80%,the effective rate of type ? b was 68.75% The effective rate was 66.67%,the effective rate of type IV was 100%,with no significant difference(P> 0.05).(4)In 101 cases of thymoma,pathological type A thymoma was 33.33% with MG,type AB with 28% with MG,type B1 with 55.56% with MG,type B2 with 68.18% with MG,B3 60% of patients with type MG,25% of type C with MG,the difference was statistically significant(P <0.05),type B2,type B3 thymoma is more likely to merge myasthenia gravis.(5)Among 101 patients with thymoma,the incidence of myasthenic crisis in stage I thymoma was 0%in Maosaoka stage,5.88% in stage II and 50% in stage III,and the incidence in stage IV was 100%,the difference was statistically significant(P <0.05),stage ?,? thymomamore likely to be associated with myasthenia gravis.(6)Among the 49 patients with thymoma and myasthenia gravis,the incidence of myasthenic crisis of type A thymoma was 0%,AB type was 14.29%,type B1 was 10%,type B2 The incidence was 20%,the incidence of type B3 was 16.67%,and the incidence of type C was 0%.There was no significant difference(P> 0.05).(7)In 49 cases of thymoma with myasthenia gravis,the incidence of myasthenia gravis in Massaoka stage ? thymoma was 0%,in stage? was 5.88%,in stage ? was 50%,in stage ? The incidence rate was 100%,the difference was statistically significant(P <0.05),stage ?,? thymoma patients more likely to have myasthenia gravis crisis.Conclusion:(1)The efficacy of thymoma resection on thymoma with MG is related to the pathological types of thymoma.The efficacy of resection on pathological type A,type AB,type B1 and type B2 thymoma resection is better,and the efficacy of type B3 and type C thymoma resection is poor.(2)The efficacy of thymoma resection on thymoma with MG is related to the Maosaoka staging of thymoma,the efficacy of resection on Maosaoka stage I and II thymoma is better,resection on stage III and IV thymoma is less effective.(3)The pathological type of thymoma is associated with the occurrence of myasthenia gravis,and thymoma with pathological types B2 and B3 is more likely to have myasthenia gravis than other pathological types of thymoma.(4)Thymoma Masaoka clinical stage associated with the occurrence of thymoma,stage?and ? thymoma more likely to be associated with myasthenia gravis,stage ? and? thymoma less likely to have myasthenia gravis.(5)Thymomas with myasthenia gravis in patients with the myasthenia gravis crisis relates with the Maosaoka clinical stage of the thymoma,stage ? and ? thymoma more likely to appear myasthenic gravis crisis,and stage ?,?,the risk of gravis myasthenic crisis Smaller.
