Objective:To study the risk factors of gastroesophageal reflux disease(GERD)in diabetic patients.Methods:Choose 229 patients who had been diagnosed as diabetes in our digestive department from January 2019 to December 2019 were selected to formulate.Gastroesophageal reflux disease questionaire(Gerdq)was used as the diagnostic basis of GERD.The total score of GERD was 8 points or more.A total of 50 diabetic patients with gastroesophageal reflux disease were selected as the case group and 179 diabetic patients without gastroesophageal reflux disease as the control group.The criteria are:1.Patients coming to our hospital for treatment due to digestive tract symptoms such as acid reflux,heartburn,chest pain,dysphagia or extraesophageal symptoms;2.The diagnosis of diabetes is based on the diagnostic criteria proposed by WHO in 1999.3.Gastroscopy may indicate reflux esophagitis or exclude peptic ulcer,gastrointestinal bleeding,tumor,and the damage of esophageal mucosa caused by physical and chemical factors such as mycotic esophagitis;4.Informed consent has been signed by patients and their families.Exclusion criteria are:1.Patients taking PPI in treatment in four weeks;2.Previous diagnosis of organic diseases such as benign and malignant tumor of digestive system and gastrointestinal operation history;Collecting gender,age,body mass index(BMI),smoking,drinking and Helicobacter pylori infection(HP),non steroidal anti-inflammatory drugs(NSAIDs),course of diabetes,Fasting plasma glucose(FPG),glycosylated hemoglobin(Hb A1C),total cholesterol(TC),triglyceride(TG),High density lipoprotein(HDL-C),uric acid(UA),FPG,Hb A1 c,TC,TG,HDL-C,uric acid,diabetic drugs,diabetic nervous system complications and other factors,after data collection,analyze by SPSS 25.0,the measurement data of normal distribution is expressed as mean ± standard deviation,Conduct independent sample t-test,the measurement data of non normal distribution is expressed by the interval between median and quartile,Mann Whitney U test,Count data in frequency,Chi square test,Single factor with statistical significance in the occurrence of gastroesophageal reflux disease and diabetes,Based on this,the regression analysis model is established,Two categories of unconditional logistic regression analysis,Identify risk factors,P < 0.05 was statistically significant.Results: 1.General data statistics:179 cases in the control group,93 women,(51.96%),86 males(48.04%),The age was 41-90 years old,The average age was 63.30 ± 8.696 years;There were 50 cases in the case group,including 26 women(52%),24 men(48%),The age was 41-81 years old,The average age was63.74±8.616 years.2.Age analysis:Divide the staff into 5 groups by age,the largest proportion of people is 61-70 years old(40%)in the case group,followed by 51-60 years old(32%),68% over 60;In the control group,people aged 61-70 account for the largest proportion(49.73%),66.48% over 60 years old.There was no significant difference in the positive rate of each age group(P>0.05).3.Single factor analysis showed that there were significant differences between the two groups in GERD,BMI,TG,Hb A1 c and diabetic nerve complications(P<0.05),There was no significant correlation with smoking,drinking,age,sex,Helicobacter pylori infection,NSAIDs,FPG,TC,HDL-C,LDL-C and UA(P>0.05).4.Multivariate logistic regression analysis showed that the risk factors of diabetic patients with GERD were Hb A1 c and diabetic neurological complications.Conclusion: Hb A1 c and diabetic nervous system complications may be risk factors of diabetic patients with GERD.Active control of blood glucose and treatment of diabetic neurological complications are of great significance for the prevention of GERD in diabetic patients.
