There are 4 million leukemia patients in China,increasing at a rate of 60,000 per year.Leukemia patients not only physically suffer from the disease pain,but also suffer psychologically.Among them,more than 90 percent of the leukemia patients have an obvious psychological burden,and more than half of the patients have hospitalization anxiety and resistance to treatment.Exploring the psychological dilemma of leukemia patients is of great significance for improving the existing intervention methods and realizing the "disease treatment and heart treatment".This article adopts a qualitative research method to understand the current situation of the psychological plight of leukemia patients through in-depth interviews and participatory observation,and further explore how the psychological difficulties arise and have an influence on the leukemia patients,so as to provide a realistic basis for the intervention of medical social work.The study found that leukemia patients with psychological difficulties are often accompanied by strong negative emotions and negative behavioral choices.Negative emotions can be divided into three categories: "fear","worry" and "resentment".Negative behaviors include avoiding medical treatment,talking about "white" color change,refusing to communicate,etc.The formation of psychological difficulties in patients with leukemia is continuously dynamic,with both individual differences and group universality.The common reason for the psychological dilemma is that many needs cannot be met.Specifically,it can be summarized as the lack of physiological needs,the inability to guarantee the security needs,the breaking of the balance state between emotion and attribution,and the loss of self-esteem needs.Leukemic patients affected by strong psychological distress will produce lower primary and secondary evaluations.Low primary evaluation is that leukemia is a threatening challenge,and eventually subside and abandon itself.Low secondary evaluation is considered that their own ability and master less resources,and eventually produce self-doubt,deepen the sense of powerlessness.This article proposes that medical social workers should conduct professional and effective intervention based on the psychological dilemma of leukemia patients.Through the intervention manual design,it provides a reference for medical social workers to deal with the psychological difficulties of leukemia patients by means of a single social group: physical adaptation,emotional adjustment,self-cognition,social support,and two special periods of home treatment and transplant preparation.
