Objective To explore psychological and social influential factors of suicide ideation and evaluate the effects of establishing School-based system of identification and support on high-risk individuals for reducing the rate of suicide ideation of medical undergraduates in Anhui Province.Methods We selected 10344 undergraduates from the First and Second grade among three medical university as subjects to evaluate psychosocial influential factors of suicide ideation. Anhui Medical University was selected as intervention group, and Bengbu Medical University and Wannan Medical University as control groups to evaluate the effects of School-based system of identification and support on high-risk individuals for reducing the rate of suicide ideation. Data were collected using Suicide Attitude Scale, acceptability of suicide questionnaire, life events Scale, Impulsiveness Scale, Aggressiveness Scale, Beck Anxiety Inventory Scale, Social Support rating Scale and a questionnaire designed by ourselves.Results 48.1% (2921/10344) of undergraduates had suicide ideation. 52.5% (2921/5564) of female and 42.9% (2053/4780) of male had suicide ideation. The comparison between the male and female for the rate of suicide ideation was significantly different(P<0.05).In baseline investigation, Multi-variable logistic regression analysis showed that grade, sex, self-control of suicide, stigma of suicide behavior, positive aspects of suicide and positive feelings, acceptability of suicide, quality of life, anxiety, aggressiveness, depression, social support,negative life events and family functioning were the risk factors for severity of suicide ideation. Grade, stigma of suicide behavior, the society importance of suicide, acceptability of suicide, quality of life, anxiety, aggressiveness, depression, social support and negative life events were the risk factors for severity of suicide ideation of the female. The status of love, stigma of suicide behavior, positive aspects of suicide and positive feelings, acceptability of suicide, quality of life, anxiety, aggressiveness, depression, social support and negative life events were the risk factors for severity of suicide ideation of the male.In follow-up investigation, adjusting severity of suicide ideation in baseline investigation and influential factors of suicide ideation, multivariable analysis showed that intervention—School-based system of identification and support on high-risk individuals could reduce severity of suicide ideation.Conclusion The undergraduates'suicide ideation is influenced by various psychological and social factors. The prevention of School-based system of identification and support on high-risk individuals can reduce rate of suicide ideation, we should complete intervention system, enhance undergraduates suicide prevention, explore a better defense mechanism.
