In this paper, questionnaires, interviews and other research methods are adopted, which has a more comprehensive and systematic analysis for the "Learning Plan Guidance" teaching model in high school mathematics. The article is divided into five parts: the first part raises the problem; the second part introduces the research ideas and research methods ; The third part introduces the domestic and Japanese related teaching models, and the theoretical basis and content for the"Learning Plan Guidance" in high school mathematics; The fourth part describes the preparation of the case study guide, content, and the basic operating procedures of this model, analyzing and researching the students'and the teachers'questionnaire; The fifth part is the meaning and some of my own experience during the implementation of high school mathematics "learning Case Guidance ".The research selected high school sophomores in Lianyungang City, focusing on the subjects of humanism, "Zone of Proximal Development theory"and constructivism as the theoretical basis, using questionnaires and interviews to conduct research, and had high school mathematics, "Case Study Guidance "the practice of teaching, the following conclusions are reached .1. The application of the teaching mode helps improve student learning and their self-learning ability.2. The application of the teaching is conducive to professional development of teachers and teaching methods'change.3. The application of the three-dimensional teaching model is conducive to teaching objectives and achieve the maximum unity of teaching.
