In the 21 st century,China's aging has started to accelerate,accompanied by aging and longevity.The changes in population structure,family structure,and people's attitudes have gradually weakened the family pension function and need to vigorously develop socialized old-age care methods,including community home-based pension and institutional pension.Affected by traditional culture and economic conditions,community home-based pension is the ideal way of supporting the elderly in the eyes of most elderly people.Not only can they continue to be cared for by their families,but they can also enjoy the services provided by the community.This is also the old-age pension advocated by the government's “9073” model.On the other hand,due to the gradual decline in organ function because of old age,older people are often accompanied by one or more chronic diseases,and some disabled and semi-disabled elderly need daily medical care and rehabilitation.However,the old-age retirement service model of “cultivation” and “medicine” has caused the elderly people to either struggle between home and hospitals,or delayed hospital discharge.This has caused the phenomenon of hospital “keep bed”,which has wasted medical resources and has not fully utilized it.Out of the advantages of community home care,it is difficult to meet the needs of the elderly for medical care,health care and ordinary care.Therefore,since 2013,the state has proposed to speed up the all-round development of“medical support” and to integrate nursing homes,residents' families,and communities into medical and health resources.This paper has learned from field surveys in some communities in Changchun that the community-based “medical support” pension service model has been gradually developed,but it is also affected by a number of constraints,such as the lack of continuity of investment,simple service facilities;poor coordination between departments,lack of industry standards;a small number of professional medical personnel,the difficulty of raising business standards;policy is not publicized,the elderly do not understand the combination of medical care and pension services;and so on.Based on this research and drawing on the experiences of other countries' communities in participating in the “medical support” pension service,and combining the specific conditions of Changchun,the paper puts forward recommendations for improving the “medical care and support”pension service in Changchun's community: under the premise of government,explore market methods and provide multiple services to provide medical care and pension services.At the same time,related departments should formulate industry standards for medical care and pension services as soon as possible.This is the institutional guarantee for sustainable development of this model.The use of Internet technology will refine service content,give full play to the role of community health service centers,and train professional elderly care service teams to meet the different “medical” and “maintenance”service needs of older people.
